Technical assistance is an important tool for health care process redesign, especially for physicians, nurses and office staff in small practices, which may not have sufficient resources including staff, time, and expertise to analyze data and map measures to processes and outcomes. During this webinar, physician panelists will share their experiences in pay for performance (P4P) demonstrations and provide guidance, tools and tips on what is takes for physician practices to succeed under P4P systems. They will also discuss recommendations from a new report, Rewarding Results Pay-for-Performance: Lessons for Medicare, http://www.hcfo.org/publications/rewarding-results-pay-performance-lessons-medicare, on the lessons learned from commercial payer demonstrations and their implications for physician practices participating in Medicare demonstrations.

Why Should My Practice Participate?

The webinar will focus on key issues associated with setting up processes to redesign patient care and capture data necessary for reporting quality measures and how to use that data at the point of care to transform care for Medicare beneficiaries. Participants in this webinar will:

  • Receive how-to guidance for integrating quality reporting and feedback into clinical practice given limited resources
  • Gain ideas about how to improve workflow and leverage tools and infrastructure
  • Learn about practical and successful care management models that can be used to transform care in small physician practices
